Extension ladders are commonly used for roof and hatch access. The only requirement imposed by OSHA is to

"extend ladders at least 3 feet above the landing to provide a handhold or for balance when getting on and off the ladder from other surfaces."

However 1991 National Safety Council study "A Scenario Analysis of Ladder Fall Accidents" unambiguously concludes that

"This antiquated, unbalanced and dangerous maneuver is the #1 cause of ladder overturning or moving."
